How much does Medicare pay for a knee replacement?
Nationally in 2024, Medicare's surgery-center rate for a total knee (CPT 27447) was $9,048 before local wage adjustment; the hospital outpatient rate was $11,940; and hospitals were paid a median $15,666 per inpatient stay (MS-DRG 470).
How much of a joint replacement payment is the implant?
CMS counts 47% of the outpatient knee payment, 50% of the hip and 61% of the shoulder as the implant (device offset).
Why do hospitals charge more than surgery centers?
Medicare pays each setting under a different fee schedule. Inpatient payments bundle the overnight stay and are adjusted for teaching, safety-net and wage costs; surgery-center rates are lower by design. The implant is priced the same.
