Where do these commercial rates come from?

From the machine-readable price files hospitals must publish under the federal hospital price transparency rule (45 CFR 180.50). We read 343 hospitals' files in 42 metro areas and keep only dollar amounts a named commercial insurer agreed to for the exact code, screened against the hospital's own Medicare rate.

Is this what I will pay?

No. These are the amounts the hospital and insurer agreed to for the facility portion. What you pay depends on your plan's deductible, coinsurance and out-of-pocket maximum; surgeon and anesthesia bills are separate.
